L Latar belakang: Sodium hypochlorite merupakan bahan pilihan utama irigasi karena kemampuan melarutkan jaringan dan efek antimikroba yang kuat, terutama terhadap biofilm. Pemakaian sodium hypochlorite tetap harus dibatasi dalam sistem saluran akar, karena efek kaustiknya pada jaringan periapikal. Ekstrusi irigasi yang tidak disengaja selama irigasi saluran akar dan gejala akibat ekstrusi telah didokumentasikan dalam banyak laporan kasus. Tujuan: Mengetahui perbedaan ekstruksi apikal saat irigasi menggunakan jarum irigasi single side-vented, double sides-vented dan modified double side-vented dengan jarak insersi 2 mm dan 3 mm lebih pendek daripada panjang kerja. Metode: 60 gigi premolar pertama rahang bawah yang telah diekstraksi dengan saluran akar tunggal. Preparasi saluran akar sampai dengan ukuran #30.07 dan dilakukan irigasi NaOCl 5% menggunakan jarum single side-vented, double side-vented, dan modified double side-vented dengan kedalaman insersi lebih pendek 2 mm dan 3 mm daripada panjang kerja. Ekstrusi diukur dengan menggunakan electronic weighing machine. Analisis hasil dilakukan uji normalitas menggunakan uji Shapiro-Wilk kemudian dilanjutkan uji two-way ANOVA dan post hoc Tukey. Hasil: Tidak ada perbedaan bermakna berdasarkan jarum single side-vented dan double side-vented pada jarak insersi jarum 2 mm dan 3 mm lebih pendek daripada panjang kerja dengan jarum modified double side-vented pada jarak insersi jarum 3 mm lebih pendek daripada panjang kerja. Terdapat perbedaan bermakna berdasarkan jenis jarum modified double side-vented pada jarak insersi jarum 2 mm dan 3 mm lebih pendek daripada panjang kerja. Kesimpulan: Pemakaian jarum modified double side-vented pada jarak insersi 2 mm lebih pendek daripada panjang kerja menyebabkan ekstrusi apikal.

B Background: Sodium hypochlorite is the irrigator of choice due to its tissue-dissolving ability and strong antimicrobial effects, especially against biofilms. The use of sodium hypochlorite should still be limited in root canal systems, due to its caustic effect on periapical tissues. Accidental irrigation extrusion during root canal irrigation and extrusion-induced symptoms have been documented in many case reports. Objective: To determine the difference in apical extrusion during irrigation using single side-vented, double sides-vented and modified double side-vented irrigation needles at insertion distance 2 mm and 3 mm shorter than WL. Methods: 60 mandibular first premolars that had been extracted with single root canals. Root canals were prepared to size #30.07 and irrigated with 5% NaOCl using single side-vented, double side-vented, and modified double side-vented needles at insertion distance 2 mm and 3 mm shorter than WL. Extrusion was measured using electronic weighing machine. The results were analysed using normality test using Shapiro-Wilk followed by two-way ANOVA and Tukey's post hoc. Results: There was no significant difference based on single side-vented and double side-vented at insertion distance 2 mm and 3 mm shorter than WL with modified double side-vented at insertion distance 3 mm shorter than WL. There is a significant difference based on the type of modified double side-vented needle at insertion distance 2 mm and 3 mm shorter than WL. Conclusion: The use of modified double side-vented needles at insertion distance 2 mm shorter than WL causes apical extrusion.
