P PT. Bakrie Metal Industries merupakan perusahaan yang berfokus pada bidang manufacturing corrugated metal product dan fabrication project. Salah satu produk yang dihasilkan adalah produk steel tub girder ramp 2. Berdasarkan data perusahaan, rejection rate atau presentase cacat produk tertinggi pada bulan Juli 2020 sebesar 7,214%. Batas toleransi perusahaan untuk rejection rate sebesar 4%. Tujuan penelitian yang dilakukan adalah meningkatkan kualitas produk dengan meminimasi kecacatan produk, meningkatkan tingkat sigma, dan mengevaluasi faktor penyebab kecacatan produk berdasarkan metode six sigma melalui tahapan DMAIC. Tahap define terdapat diagram SIPOC produksi dan jenis cacat pada CTQ adalah slag inclusion, porosity, dan incomplete penetration. Tahap measure menggunakan perhitungan peta kendali p, peta kendali c, DPMO, dan tingkat sigma. Tingkat sigma memperoleh nilai sebesar 3,33 sigma sebelum perbaikan. Tahap analyze berisikan analisis sebab dan akibat cacat produk menggunakan metode diagram pareto, metode fishbone diagram, metode Failure Mode and Effect Analysis (FMEA), dan metode Fault Tree Analysis (FTA). Penyebab kegagalan tertinggi adalah slag inclusion sebesar 52,9% dan porosity sebesar 28,2%. Berdasarkan nilai RPN dan nilai probabilitas tertinggi, efek penyebab potensial proses interpasss cleaning pada lasan tidak sempurna dengan nilai RPN sebesar 486 dan nilai probabilitas top event sebesar 0,71, efek penyebab potensial welder melakukan kelalaian saat proses pengelasan dengan nilai RPN sebesar 432 dan nilai probabilitas top event sebesar 0,55, dan efek potensial penyebab potensial terdapat zat pengotor pada lasan sebesar 384 dan nilai probabilitas top event sebesar 0,48 . Tahap improve memberikan usulan perbaikan berupa menggunakan mesin gerinda dan pembuatan checksheet beserta instruksi kerja pengisian checksheet saat proses interpass cleaning, pembuatan SOP proses pengelasan, dan checksheet serta instruksi pengisian checksheet inspeksi material. Tahap control tidak dapat dilakukan implementasi usulan perbaikan dikarenakan adanya covid-19.

Based on the RPN value and the highest probability value, the effect of the potential cause of the interpass cleaning process on imperfect welds with an RPN value of 486 and top event probability value of 0.71, the effect of potential cause of a welder doing negligence during the welding process with an RPN value of 432 and a probability value the top event is 0.55, and the effect of potential cause of there are impurities in the weld with and RPN value of 384 and top event probability is 0.48. The improvement stage provides suggestions for improvements in the form of using a grinding machine and making checksheets with instruction during the interpass cleaning process, making SOPs for the welding process, and material inspection checksheets with instruction.Control stage cannot be implement because the outrage of covid-19.
