P T. Cahaya Buana Intitama is a company engaged in the manufacturing of plastic furniture products. The types of products that have the most production demand are BCBC type CRS 5.2C 4MU 1DK types. The company has quality problems, based on historical data in June 2019 - August 2019 has an average percentage of defects of 2,006% with a target of 1.5% of company defects. This study aims to identify the main root causes of failures in the injection process and can provide recommendations for gradual improvement in order to improve product quality. The stages of the study were carried out by the SPC and Fuzzy FMEA methods. The output produced in the form of the highest FRPN value with the category Very High. The FRPN value which is categorized as Very High produces a value of 883 with the cause of uneven mold flow and 821 with the cause of the inappropriate pillow size. The results of the proposed improvements given include: providing leeway time, training, monitoring operator performance, checking the mold condition check, mold installation instructions, monitoring barrel temperature, designing guidelines for barrel temperature use, setting the injection machine for charging time, and designing guidelines for range sizes cushion The results after the implementation of the percentage of short shots defects was reduced from 3.52% to 1.03% and the value of the sigma level increased from 3.68 to 3.78 sigma
