D Dalam proses produksi balpress mengandalkan keterampilan operator, namun tak jarang pula aktivitastersebut menyebabkan terjadinya human error. Hal tersebut ditunjukan dengan kerusakan produk balpress.Dalam melakukan penelitian ini digunakan metode Systematic Human Error Reduction and PredictionAnalysis (SHERPA), Human Error Assessment and Reduction Technique (HEART), dan Fault TreeAnalysis (FTA). Pada hasil identifikasi metode SHERPA ditemukan 11 aktivitas operator yang berpotensihuman error. Identifikasi metode HEART menunjukkan nilai Human Error Probability tertinggi sebesar0,9471 pada kegiatan pembersihan mesin press. Pada tahap terakhir menggunakan metode FTA. MetodeFTA menghasilkan nilai probabilitas kejadian tertinggi pada basic event sebesar 0,29 tidak terdapat SOPterkait pembersihan mesin. Kemudian pada lingkungan kerja yang panas dan berdebu sebesar 0,25. Padaposisi kerja operator yang kurang baik sebesar 0,17. Usulan perbaikan adalah membuat StandarOperasional Prosedur dan checklist. Standar Operasional prosedur dan checklist dibuat pada prosespembersihan mesin, inspeksi balpress, dan pembersihan lingkungan kerja. Usulan perbaikan yangdiberikan dilakukan validasi dengan analisis resiko, mempertimbangkan probabilitas dan nilaiseverity.Tahap validasi dilakukan untuk mengetahui probabilitas kejadian dan tingkat keparahan apabilaperusahaan tidak menerapkan implementasi yang diberikan. Dari setiap usulan perbaikan yang diberikan didapatkan nilai probabilitas dan severtity dari masing-masing usulan.
B Balpress production process relies on operator skills, but not infrequently these activities causehuman errors. This is indicated by the damage to the balpress product.. A hot and dusty work environment produced a probability of 0.25 and0.17 for the operators' ineffective working positions. The proposed improvements were to prepare StandardOperating Procedures and checklists. Standard operating procedures and checklists were made on theprocess of cleaning machines, balpress inspections, and the cleanliness of work environment. Thesemethods had been validated with risk analysis, considering the probability and severity value. Thevalidation stage was carried out to determine the probability of occurrence and severity if the company didnot implement the implementation given. From each proposed improvement, the probability and severtity values of each proposal were obtained
