Hazard identification risk assessment determine control (hiradc) untuk minimasi bahaya dan risiko kecelakaan kerja pada pengoperasian lathe machine
P ada kegiatan service peralatan eksplorasi minyak bumi dan gas di PT. Revolutek Dananjaya Mandiri khususnya pada pengoperasian lathe machine memiliki potensi bahaya dan risiko yang dapat terjadi. Pada penelitian pendahuluan ditemukan sumber bahaya dan potensi kecelakaan kerja. Kecelakaan yang ditemukan adalah pekerja terluka karena terkena serpihan gram, terjatuh akibat lantai produksi yang licin, dan terganggu pernafasannya akibat udara mengandung debu. Penelitian ini bertujuan untuk meminimasi bahaya dan risiko dalam proses pengoperasian lathe machine di PT. Revolutek Dananjaya Mandiri. Diperlukan upaya pengendalian bahaya dan risiko melalui penilaian risiko, identifikasi bahaya, serta penentuan pengendalian menggunakan metode Hazard Identification Risk Assessment Determine Control (HIRADC). Variabel potensial hazard ditentukan dengan pendekatan Job Safety Analysis (JSA). Variabel deviation ditentukan dengan pendekatan Hazard and Operability (HAZOP). Hasil penelitian menunjukkan adanya 14 aktivitas pekerjaan dengan 22 hazard, dan menghasilkan 38 rekomendasi pengendalian bahaya pada proses machining dengan lathe machine. Pengendalian bahaya dan risiko terdiri dari Substitusi, Pengendalian Keteknikan, Tanda bahaya/peringatan, dan Alat Pelindung Diri/Personal Protective Equipment. Semuanya sudah diimplementasikan dan hasilnya adalah penurunan skala risiko kecelakaan kerja rata-rata sebesar 60%. Semua hazard yang telah diidentifikasi telah masuk pada kategori low risk
I n the service activities of oil and gas exploration equipment at PT Revolutek Dananjaya Mandiri, especially in the operation of lathe machines, some potential hazards and risks can occur. In preliminary research, sources of danger and potential work accidents were found. The accidents found were workers injured by gram splinters, falling due to slippery production floors, and breathing problems due to air-containing dust. The purpose of this study is to minimize hazards and risks in the lathe machine operation process at PT. Revolutek Dananjaya Mandiri. Hazard and risk control efforts are required through hazard identification, risk assessment, and determination of controls using the Hazard Identification Risk Assessment Determine Control (HIRADC) method. Potential hazard variables are determined with the Job Safety Analysis (JSA) approach. Deviation variables were determined using the Hazard and Operability (HAZOP) approach. The results showed 14 work activities with 22 hazards and produced 38 recommendations for hazard control in the machining process with a lathe machine. The hazard and risk control consisted of substitution, engineering control, danger/warning signs, and personal protective equipment. All of them have been implemented and the result is a decrease in the risk scale of work accidents by an average of 60%. All hazards that have been identified have been categorized as low risk
