PT. Bukaka Teknik Utama merupakan perusahaan yang menghasilkan alat berat.Salah satu plant yang berada di PT. Bukaka Teknik Utama adalah divisi Machining andGear Shop yang memproduksi vessel pressure. Berdasarkan data kecacatan dan orderproduksi pada bulan juli – september 2011 didapatkan kegagalan proses sebanyak 4 unitdari 24 kapasitas produksi atau sebesar 16.67% persen cacat dalam proses painting.PT. Bukaka Teknik Utama divisi Machining and Gear Shop ingin mengetahuisumber dan penyebab kecacatan pada proses produksi vessel pressure. Untuk itu,dilakukan analisa serta perbaikan lebih lanjut terhadap proses produksi menggunakanmetode fuzzy FMEA untuk mengidentifikasi faktor penyebab kegagalan dan menghitungnilai FRPN tertinggi sebagai prioritas penyebab kecacatan utama. Dari hasil pengolahandata dengan menggunakan fuzzy FMEA, Diketahui cacat yang terbesar adalah pada prosespengecetan warna dengan menggunakan spray gun dengan nilai FRPN 599 dan didapatkanpula nilai FRPN terendah dengan nilai 313 pada proses polishing dimana mempunyai efekkegagalan berupa warna cat memudar karena proses polishing yang terlalu berlebihan ataucompound terlalu banyak.Kegagalan proses terjadi akibat jangkauan pengecetan yang sulit terjangkau danteknik aplikasi pengecetan yang belum benar. Untuk mencegah atau minimasi kecacatan,maka dibuatlah alat bantu “jig tempat pengecetan†yang diharapkan dapat membantumempelancar proses painting vessel pressure.Monitoring form yang bertujuan untukmemberikan prosedur yang tepat dalam pengecekan produk vessel pressure pada prosespengamplasan

Based on the data of the flows and production order from july – september2011, we got the result of failure of process, as much as 4 units of 24 production capacityor 16.67% of defect in the painting process.PT. Bukaka Teknik Utama, or to be more precise the Machining and Gear Shopdivision wanted to know the source and the cause of the given flaws at the productionprocess of vessel pressure. For that, there hold an analysis and also further improvementsabout the production process using the Fuzzy FMEA to identity the factors that contributeto the failure and to calculate the highest FRPN number as the main factor of the givenfailure. From the result of data sampling with the Fuzzy FMEA, we know that the biggestmisexecution happened on the painting process which used an air spray with the level ofFRPN 599. And we got the lowest FRPN with a value of 313 in the polishing process wichhave paint color fading as effect of failure beacuse of too excessive polishing process ortoo much compoundThe failure happened at then cause of the difficulty in spraying to reach somecertain spots and the application of spraying technique which was poorly conducted. Toprevent or to minimize the faults, thus was made an aiding device which is called “paintingarea of jig†which we are hoping can help to accelerate the process of vessel pressurepainting.Monitoring form that aims to provide appropriate procedures in checking theproduct of vessel pressure on the sanding process
