P Pada Plan WTM 16 di PT. XYZ terjadi kecelakaan kerja yang dikarenakan human error, dimana operator tidak mengoperasikan mesin sesuai prosedur. Tujuan dari penelitian ini yaitu mengidentifikasi upaya pencegahan yang dilakukan untuk mengurangi human error pada Plant WTM 16 di PT. XYZ. Metodologi yang digunakan yaitu metode Job Safety Analysis (JSA) untuk mengidentifikasi potensi bahaya disetiap aktivitas pekerjaan. Metode Cognitive Reliability and Error Analysis Method (CREAM) yang digunakan untuk predictively (memprediksi kemungkinan human error) dan retrospectively (menganalisa dan menghitung error). Metode Hazard Identification, Risk Assessment and Risk Control (HIRARC) yang digunakan untuk menentukan upaya pengendalian agar dapat mengurangi bahaya yang teridentifikasi. Dari hasil penelitian ini diketahui resiko bahaya dari setiap aktivitas yang dilakukan operator menggunakan metode JSA dan diketahui penilaian probabilitas kegagalan dari setiap elemen kerja yang mengalami kegagalan kognitif dengan metode CREAM. Usulan yang diberikan yaitu menerapkan Standar Operasional Prosesdur (SOP). Hasil Cognitive Failure Probability (CFP) sebelum dilakukan perbaikan sebesar 0,7 dan setelah dilakukan perbaikan hasil Cognitive Failure Probability (CFP) menjadi 0,1.

I In Plan WTM 16 at PT. XYZ there was a work accident caused by human error, where the operator did not operate the machine according to the procedure. The purpose of this research is to identify prevention efforts to reduce human error at Plant WTM 16 at PT. XYZ. The methodology used is the Job Safety Analysis (JSA) method to identify potential hazards in each work activity. The Cognitive Reliability and Error Analysis Method (CREAM) method is used predictively (predicting the possibility of human error) and retrospectively (analyzing and calculating errors). The Hazard Identification, Risk Assessment and Risk Control (HIRARC) method is used to determine control efforts in order to reduce the identified hazards. From the results of this study it is known that the hazard risk of each activity carried out by the operator using the JSA method is known and it is known that the probability of failure of each work element experiencing cognitive failure is known using the CREAM method. The proposals given are implementing Standard Operating Procedures (SOP). The Cognitive Failure Probability (CFP) result before the repair was carried out was 0.7 and after the repair was carried out the Cognitive Failure Probability (CFP) result became 0.1.
