P PT. Argo Pantes Tbk merupakan salah sntu perusahaan manufaktur di Indonesia yang bergerak di bidang tekstil. Salah satu produk yang dihasilkan yaitu kain, yang terdiri dari kain grey dan kain motif. Kain motif memiliki persentase cacat lebih besar dibandingkan kain grey yaitu 8,07%. Karena tingginya persentase cacat tersebut maka perusahan perlu melakukan peningkatan kualitas terhadap produk yang dihasilkan. Pemecahan masalah dilakukan dengan menggunakan metode six sigma melalui tahapan DMAIC (Define, Measure, Analyze, Improve, dan Control) dengan menerapkan Failure Mode and Effect Analysis (FMEA) pada tahap analisa. Saat ini perusahan memiliki nilai DPMO sebesar 9512 dengan ting)cat sigma 3,85 sigma. Untuk mencapai tingkat sigma yang diharaplcan, dilakukan analisa dan perbaikan terhadap proses produksi dengan menggunakan tabel FMEA untuk mengetahui penyebab terjadinya kecacatan atau kegagalan yang menjadi prioritas penanganan masalah. Berdacarkan analisa tersebut diketahui penyebab kegagalan yaitu operator kurang teliti saat melakukan penyisiran dengan nilai RPN 140, operator kurang teliti saat memasukan benang ke lubang dropper dengan nilai RPN 120, dan kesalaahan pada saat pembukaan mulut lusi dengan nilai RPN 96. Untuk meminimasi penyebab terjadinya kegagalan maka diberikan usulan perbaikan agar dapat meningkatkan kualitas produk. Usulan perbaikan yang diberikan yaitu memberikan fasilitas untuk menunjang kenyaman operator diantaranya memperbaiki atau mengganti kipas angin yang rusak dengan yang baru, membuat langkah kerja mengenai pemasangan beam yang akan di cucuk dan disisir, membuat daftar tugas dan kewajiban operator reaching atau cucuk, membuat langkah kerja kerja pada proses tenun, dan membersihkan mesin dan sisa lcanji sebelum dan sesudah proses pengkanjian. Setelah dilakukan implementasi usulan perbaikan diperoleh penurunan nilai DPMO menjadi 8054 dan peningkatan nilai sigma menjadi 3,91.

The quality of the resulting cross table became one of the main factors that are the focus of the company to face competition, but in the company actual experienced difficulties in addressing the problem of product quality. In the process of table cross production has found defect caused by failures in the production process began in the early stages of raw material receipt, process in the oven, the process of cutting, smoothing, bamboo coating process until the final packaging process. This study aimed to help the company in the identification of a process failure occurs. Identification of the failure process is made by observation of the production process of cross table and by interviews the company, followed by the formation of Failure Mode and Effect Analysis (FMEA). This method was expected to analyze and find out the cause of the failure process so that corrective action can be determined to address the problems faced by the company. While in handling the priority of problems with FMEA method performed to obtain the ranking of problems priority that should be corrected based on the highest risk. The highest RPN valueshas occur in the process of oven with RPN value of 288, the process of cutting with RPN value of 384, and the bamboo coating process with RPN value of 256. After identifying the problem with FMEA, in addressing the problems made by constructing of knowledge model, Knowledge model is expected to be a supporting tool as a source of information for workers in the company to facilitate the searching information and decision-making processes in dealing with various problems on the production process without having to deal directly with the experts. In building a model of knowledge made the determination of solution to be applied to model the relevant knowledge. Based on analysis carried out, then for overcoming problems in the process of oven is done by constructing composition ladder, atribute ladder and process ladder. While for the process of cutting is done by building process ladder for SOP of ripsaw machine and banndsaw machine, dan SOP of marking gauge. Proposed recommended improvements to the company performed in the form of SOP for the oven process and cutting process. Purpose of making this SOP in order to avoid the same mistake with the problems facing the company during this.
